Wound Assessment and Management: An Interprofessional Approach

A wound is clinically defined as a damage or disruption of the cellular, anatomical, and/or functional continuum of living tissue. This disruption triggers a complex biological response that can be overwhelmed by systemic comorbidities, particularly in the aging population. In the United States, millions of individuals—with a disproportionate impact on those aged 65 and older—suffer from chronic, non-healing wounds. These wounds impose a significant burden on both the healthcare system and patient quality of life. Because clinical perspectives on wound therapy can vary significantly based on individual experience, adopting a standardized, evidence-based protocol for the interprofessional team is essential to minimize complications and enhance patient outcomes.

1. Initial Wound Assessment

The initial assessment serves as the baseline for all future clinical decisions. It must be systematic, thorough, and documented with precision.

  • Patient History: Clinicians must evaluate the „host” by identifying systemic factors that impede healing, such as diabetes, nutritional deficiencies, peripheral vascular disease, and current medication use (e.g., corticosteroids or anticoagulants).
  • Wound Etiology and Classification: Determining the cause is mandatory. Is the wound a result of pressure (ischemia), venous stasis (venous hypertension), arterial insufficiency, or trauma? The classification guides the subsequent management strategy.
  • Wound Bed Evaluation: Using the TIME or TIMERS framework, the clinician must document:
    • Tissue: Percentages of granulation, slough, or necrotic tissue.
    • Infection/Inflammation: Clinical signs of bioburden.
    • Moisture: Exudate type and volume.
    • Edges: Presence of undermining, tunneling, or epibole.

2. Types of Wounds

Broadly, wounds are classified into two primary categories based on their healing trajectory:

  • Acute Wounds: These result from a specific injury (e.g., surgical incision, laceration, or burn) and typically follow an orderly and predictable healing process. They usually progress through the four phases of healing—hemostasis, inflammation, proliferation, and maturation—without significant deviation, provided the patient is healthy.
  • Chronic Wounds: These are wounds that fail to proceed through the orderly healing process, often remaining in a prolonged inflammatory phase. These wounds typically result from underlying pathological processes such as vascular disease, diabetes, or repetitive pressure. They are characterized by a stalled healing cycle, high bioburden, and the presence of biofilms, requiring complex, long-term intervention.

3. Potential Complications in Wound Care

Complications in wound care often arise from either misdiagnosis of the wound type or failure to address systemic barriers to healing. Common complications include:

  • Infection and Biofilm: Chronic wounds are frequently colonized by bacteria, which can progress to deep-seated infection, cellulitis, or osteomyelitis. Biofilms—structured communities of bacteria—are a major cause of stalled healing.
  • Maceration and Desiccation: Failure to manage moisture balance can lead to either maceration (skin breakdown due to excessive moisture) or desiccation (cell death due to a dry, non-viable wound bed).
  • Dehiscence: The reopening of a surgical wound, often due to tension, poor nutrition, or underlying infection.
  • Chronic Pain: Often underestimated, pain significantly impairs a patient’s mobility and adherence to care, further complicating the healing process.

4. Optimal Wound Management

Management strategies must be tailored to the wound’s classification and the patient’s overall health.

  • Debridement: The removal of devitalized or necrotic tissue is critical to remove the biofilm and promote the migration of healthy cells. The method (autolytic, enzymatic, or sharp) must be chosen based on the wound’s specific needs and the clinician’s scope of practice.
  • Moisture Balance: Utilizing appropriate advanced dressings—such as foams for high-exudate wounds or hydrogels for dry wounds—creates the optimal environment for cellular activity.
  • Offloading and Compression: For pressure injuries, the pressure must be eliminated via specialized support surfaces. For venous ulcers, medical-grade compression therapy is the gold standard for reducing edema and venous stasis.
  • Interprofessional Collaboration: Optimal care relies on the input of nurses, physicians, physical therapists, and nutritionists. A multidisciplinary approach ensures that local wound care is supported by adequate systemic nutrition, blood sugar regulation, and mobility support.

By shifting from subjective, experience-based care to a structured, interprofessional assessment protocol, clinicians can ensure consistency, reduce the prevalence of complications, and significantly improve the healing rates of both acute and chronic wounds.
